Step 1- Review the benefits and features of both levels of coverage and decide if the BASIC or PREMIER level of coverage is best suited for you or your family. Remember, both plans offer 24 /7 coverage. Compare benefits here.
Step 2- Payment can be made by submitting the annual premium amount with your application, or by monthly bankdraft. If you choose automated monthly bankdrafting, you will need to submit the authorization form and a sample voided check with your completed application. The bankdraft authorization form can be downloaded here. Just sign the top and bottom. Again...send entire annual premium OR signed bankdraft form and a voided check.
Step 3- Applications must be properly filled out. The application is available online here. If you have difficulty obtaining the online version, contact us and we will mail you the application. The yellow or shaded areas must be completed. A primary beneficiary must be filled in, since the policy includes a death benefit. And last, the dependents section must be filled out only if you are purchasing the Family policy.
